General Information
Normal business hours are from 8:30 AM to 5:00 PM Monday Through Friday. Delivery and pick up in the Chicago area is available for a nominal charge during working hours. Special pick up and receiving arrangements can be made upon request.
Minimum rental period is one day and minumum rental charges of $50.00 exclusive of tax,
freight, or damage waiver. A one-day rental fee is charged when equipment is returned the next day by 10:00 AM. Rental equipment shipped out of the greater Chicago metropolitan area may be subject to a two-day minimum rental charge.
With few exceptions, early pick up may be made after 3:00 PM the day prior to the rental day at no extra charge. Saturday is a billable day - Sunday is not.
Weekly rates are three times the daily rate. Weekly rates apply only when made in advance. Extensions are considered new rental periods.
Reservations are suggested. We reserve the right to charge rental fees for cancellations made without reasonable advance notice.
All shipments of rental equipment shall be made either freight collect or prepaid with freight fees and carrier insurance charges added to the invoice. All equipment shall be returned freight prepaid, FOB destination.
All equipment is rented in good condition and is to be returned in good condition, with ordinary wear and tear allowed. Fletcher Chicago, Inc. assumes in good faith that operators of rented equipment are knowledgeable and experienced in its use.
Shipments are FOB our warehouse. The Lessee is responsible for the equipment once it is accepted by a common carrier.
In the event any part of the equipment is destroyed, damaged, lost or stolen during the rental period, the Lessee shall continue to pay rent until the equipment is replaced or repaired.
The Lessee will be given the opportunity, and is encouraged, to inspect and test the equipment prior to its rental. Thereafter, the Lessee assumes full responsibility for all items until returned. Returning the equipment does not waive possible charges for abnormal maintenance and/or repair.
Credit cards are accepted, however, the actual card must be processed at Fletcher Chicago. In lieu of processing the card at our facility, credit cards may be processed via telephone provided that such use is confirmed via fax that includes a copy of the card and a signed statement authorizing use of the card for payment of the specific obligation at Fletcher Chicago, Inc.
In the event the signatory of a credit card is not the same person named on the card, written authorization of its use must be obtained prior to its acceptance.
At no time will rental prices accrue toward the purchase of any rental equipment.
Spare lamps and batteries are provided with certain rentals. No charge is assessed if the dead lamps or batteries are returned with the rental.

Credit Requirements
Credit is granted upon completion of an account application and receipt of favorable references. A personal guarantee may be required. Allow five working days for processing.
Terms are net 30 days for established accounts.
New accounts get 15-day terms and are upgraded to 30 days once a satisfactory payment performance is established.
Cash customers provide a deposit and pay the complete rental fee at time of rental. Visa, MasterCard, American Express and Discover are accepted, subject to the aforementioned requirements.
Long-term productions are required to provide a minimum of two weeks advance rental and are billed on a regular basis with 7-day terms for the duration of the project.
All additions to the original or subsequent rental equipment bill of material are considered new rentals and must also have an advance payment. Insurance Requirements
Insurance Requirements
Fletcher Chicago requires the Lessee to obtain adequate insurance coverage for the replacement value of the Equipment and for injuries to person or property resulting from the use or the possession of the Equipment during the Rental Period (until returned to Fletcher Chicago).
Certificate of Insurance must be in house
before equipment may leave the building. It shall be the customer’s
responsibility to obtain and present the certificate to the company.
The Lessee must provide a current Certificate of Insurance, issued by its Insurance Agent specifying:

The name, address and phone number of the Agent or Producer

The Lessee name must appear as the Named Insured
-
The name of the Insurer (i.e. Insurance Company ) affording Coverage
-
General Liability Limits of at least $ 1,000,000 per occurrence

Fletcher Chicago Inc. is named as an Additional Insured with regards to Liability Coverage.

Fletcher Chicago Inc. is named as a Loss Payee for full replacement cost with regards to Equipment.
Insurance covering leased equipment is generally noted in the Other Section on most Insurance Certificate Forms:

The Type of Insurance is an Equipment Floater for Leased and Rented Property Or it might be called Equipment Floater - Property of Others or bailees.

The amount or limit of insurance -- should be for all of the rental equipment from all companies on the project (at replacement cost)

Specify the amount of the deductible for the Leased and rented Equipment
The special provisions section must additionally specify:

Coverage Form should be Special Form ( Not named perils). The was previously called All Risk

The leased equipment is on a replacement cost basis

Covers theft of equipment from locked cars

Covers rented equipment in transit
-
Worldwide coverage -- if necessary
An actual signature by the agent is required to certify the form
In lieu of an
insurance certificate, Damage Waiver will be offered to customer at a
rate of 30% of the net value of the order. Damage
Waiver is only available to rental orders that have a total replacement value of $10,000
or less. The
deductible for damage waiver is $2,500. The customer must have a cash
deposit, a credit card authorized for the deductible amount, or approved credit
established at Fletcher.
Damage
waiver will not be offered for 16mm and 35mm film cameras or accessories nor
any PL mount lenses associated with f
ilm cameras.
Examples
of cameras eligible for damage waiver are as follows: DVX-100A, PMW-EX1,
HVR-V1U, and HVX-200.

Insurance is maintained at the Lessee's own expense. Lessee's coverage is primary and non-contributory.

Homeowners Polices do not cover Rented Equipment. Companies that provide appropriate coverage include: Chubb, Hartford, Fireman's Fund, AON, Gulf, and Vigilant. Check with your Independent Agent.

If Renting the Production Offices @ 1010 N. North Branch, Lessee must show our additional interest as Lessor of Premises and show proof of Workers Compensation and Employers Liability Insurance.
